Adoption Developmental and Behavioral Pediatric Specialist Evaluation
(States: TX, OK, PA, GA, WI & NV)

3 hour telehealth visit for adoptive parent(s) and adopted child with an adoption expert
Previsit: Review of outside records uploaded to electronic health record (3 hours)
- Child welfare redacted files
- Orphanage records
- Adoption documents
- Previous evaluation reports by psychologists and psychotherapists
- School records, including special education testing and Individualized Education Program (IEP)
- Previous speech therapy, occupational therapy, ABA therapy, and physical therapy evaluation reports.
Hour 1: Comprehensive Developmental-Behavioral History with family
Hour 2: Developmental-Behavioral testing with child
Hour 3: Feedback and Recommendations with family
Comprehensive report uploaded to electronic health record (2 hours preparation time)

In-home evaluation for any child ages 3-16 years with concerns for developmental delays, developmental disabiities, and/or behavior conditions.
Pre-visit document review (2 hours):
- Review school records
- Review other evaluation reports
Pre-visit: Family will complete online developmental and behavioral questonnaires
Hour 1: Comprehensive Developmental-Behavioral Pediatric history and review of questionnaires
Hour 2: Standardized developmental testing
Hour 3: Feedback with recommendations
Comprehensive written report will be uploaded to patient portal or mailed upon request for a fee (preparation time 1 hour)
**Up to 30 mile radius for additional fee**

Duration: 3 hours Fee: $1500

2-hour Developmental-Behavioral Pediatric evaluation for any child ages 4-15 years with developmental concerns or behavior challenges
Previsit: (1 hour prep time)
- Dr. Spinks-Franklin reviews previous evaluations and school records.
- Parents/guardians complete online questionnaires
During visit:
- Complete developmental-behavioral history
- Standardized developmental testing
- Feedback and recommendations
Complete written report will be uploaded to the electronic health record (1 hour prepration time) or mailed for an additional fee

Duration: 2 hours Fee: $1200

Assistance with planning your child’s future after high school (for adopted and non-adopted youth)
For students ages 15 and older with Intellectual and Developmental Disabilities (IDD)
Review of school Individualized Education Program (IEP) and special education testing reports (1 hour)
Discussion of guardianship, shared decision-making and power of attorney options
Complete a structured, standardized interview of independent living skills (1 hour)
Discuss recommendations for post-secondary educational and occupational planning
Guardianship Physician Form will be completed and sent to the family or lawyer. (30 minutes)

Duration: 2 hours Fee: $750

Review my developmental evaluation results and recommendations for the patient you referred to me.
Provide Developmental-Behavioral Pediatric Expert guidance
- Discuss patient cases regarding developmental delays or behavior concerns
- Review psychopharmacology questions and management
- Discuss special education services and ways to support families
- Identify ways to make your practice culturally ressponsive to the families you serve.
